Registration for Heidelberg Middle School

Registration is a two part process

First, complete immunization forms. These forms can be found on the HMS school web site. Link to Nurse Web Site is above the registration link then click on immunization requirements

Second complete the packet of forms listed below. They should be completed in order. There is also link to on-line registration at the overall DoDDS registration site (at bottom of this web page) for those parents that are no currently in the local area but it can be confusing since it has all forms for all schools. If in the local area simply fill out the below forms then come in to the Registrar office at the school to complete the registration process.

Checklist for new students

The following 8 documents below are the registration packet at HMS. These forms can be downloaded or picked up at the school Registrar office. Please read all the instructions before completing forms.

1.   600 Form (2 pages) (This form can be filled out online but must be printed because a parent signature is required at HMS before the registration process is complete.)
2.  Registration Questionnaire 910-R (Sponsor Category, if not sure see below)
3.   Race/Ethnicity/Home Language Form, ESL Home Language Survey (2 pages)
4.   Registration Data Card (One per child)
5.     Weapons/Prohibited Substances/Anti-Bullying Policy (Parent signature required) & Student Computer/Internet Access Agreement (Parent signature required) (2 pages)
6.    Publicity Permission Form (Parent signature required)

Core courses for all students are math, language arts, science, and social studies. Some electives classes can be year-long courses, semester or quarter. Discuss the elective choices with your child and bring the form with you to register at the school.

School registration is not final until forms have been signed and supporting documents (i.e. orders) are brought into the Registrar office at Heidelberg Middle School..

Copy of Orders must be included to complete registration. See below for category details.

   
Category 1: Command-Sponsored DoD Employees: Space Required/Created, Tuition Free
A-E: Military : Extension orders (or) ORB / ERB showing current deros (or) Form 803
F & J:GS Civilians/ NAFI : Extension orders (or) Form 802.
G & H: US Contractor & FMS : Central billing letter for SY 07-08, TESA (or) sponsor ID, and student birth certificate.

Category 2: Federally Connected Personnel: Space Available, Tuition Paying
A: Other US Government organizations: Extension orders (or) Form 802, and student birth certificate.
B: ½ time NAFI/APF : Form 802, student birth certificate.
C: US Interest : Reserve activation orders, DLM orders, 803, (or) 802.
D: Foreign Service : Allied forces orders

Category 3: Non-Command-Sponsored DoD: Space Available, Tuition Free
A-E: Military & Activated Reserves ³ 180 days : Extension orders, student birth certificate, and marriage certificate if student is a dependent by marriage.
F-P: NAFI/APF sponsor living apart, special case : please contact registrar by email.

Category 4: Other Non-Federally connected: Space Available, Tuition Paying
A-D US and Foreign Nationals : please contact registrar by email.
*Note: In Loco Parentis : in cases of guardianship or power of attorney, please contact the registrar.
***Space Availability is determined approximately one week before school starts***
****Tuition must be paid before the student can attend school****
